Course/Module description:
The course is designed to teach the theoretical and practical models of case management for older adults in the community.

Course/Module aims:
The student will get acquainted with the role of the geriatric care manager, acquire tools for implementing the care care process in a variety of health conditions in collaboration with a range of professionals and frameworks within the community.

Learning outcomes - On successful completion of this module, students should be able to:
At the end of the course the student will be able to:
1.Describe the theoretical and practical models of case management for promoting occupational health of older adults
2.Identify and evaluate the needs for case management in critical junctions
3.Be oriented to the rights and various services in the community in order to consider options for care.
4. Communicate optimally with service providers and health care professionals in order to carry out their treatment plan effectively.
5.Implement case management tools for evaluating, designing a treatment plan, coordinating, monitoring progress and measuring outcomes.

Teaching arrangement and method of instruction: Frontal lecture, peer-based learning in small groups, and applied practice.

3.Implementing coordinated interventions with relevant individuals/services and professionals
4.Family centered practice: providing access to information; consultation and advocacy
5.Supervision for caregivers
