Course/Module description:
This course focuses on teaching experience alongside theoretical models and advanced assessment of the chronic patient case management and family in the community by Clinical Nurse Specialist. The course will discuss the chronic patient needs and will base his clinical skills needed to do the advanced clinical issues of continuity of community care. Highlighted clinical and organizational dimensions of quality of care, risk management, cost-effectiveness, legal aspects and critical thinking, evidence-based intervention

Learning outcomes - On successful completion of this module, students should be able to:
At the end of the course the student will demonstrate knowledge and skill;
1. analyzing components of chronic patient case management in the community by: social and economic trends, transition of care and health services.
2. identifying the needs of chronic patients and their families, a family nursing diagnosis.
3. identifying support systems and professional resources (formal and informal) required to plan for the complex chronic patient care and his family complex.
4. Assess the unique needs from the chronic patient's perspective and the role of the clinical nurse specialist case manager to achieve quality care and risk management.
