Course/Module description:
Assimilation of the relevant clinical knowledge and integration of evidence-based medicine with therapeutic work routine in the ward.
A combination of the theoretical and practical knowledge learned so far with acquired skills, including: familiarization with the medical chart and interpretation of laboratory tests, medicinal anamnesis, providing a medicinal explanation to patients and staff, accompanying the patients, improving the patient's satisfaction and quality of life, and matching the optimal medication for the individual patient, in terms of effectiveness and safety.
All this while maintaining the principles of medical confidentiality, respect for others and leading a suitable conversation with the recipients.
Course/Module aims:
Improvement of the student's abilities to: reviewing a medication profile, medical follow-up, and perform medication reconciliction process; makeing recommendations to the care team (doctors, nursing staff, nutritionists, etc.) regarding the medication regimen; active participating in rounds in the department; taking medication anamnesis from the patients; detecting interactions between medications; monitoring medication and giving follow-up recommendations; monitoring side effects; reviewing medication dosages and administration regimen according to the function of the eliminative organs; documentating of clinical interventions and recommendations. Providing medication advice to the patient before discharge from the ward, telephone follow-up after discharge (optional). Performing the medicinal reception and presenting the patient to the staff, assessing acute and chronic problems, identifying untreated indications and unindicated medications, identifying medication side effects and significant drug interactions, ability to identify errors in the continuity of care in the transition between the hospital and the community.
Learning outcomes - On successful completion of this module, students should be able to:
Upon successful completion of the course, the student will be able to:
1. Understand professional terms learned in the basic sciences courses including anatomy, pathophysiology and in clinical studies.
2. Collect medication information relevant to the patient's problems.
3. Identify drug related problems .
4. Understanding laboratory tests and auxiliary tests. Interpretation of laboratory tests with an emphasis on electrolytes, kidney functions, liver functions, thyroid functions, acid-base balance, endocrine disorders.
Knowledge of the different types of ancillary tests/imaging and matching them to different medical conditions.
5. Understanding common therapeutic procedures such as plasmapheresis and hemodialysis.
6. wording recommendations for changing drug treatment, using accepted medical language.
7. Follow up of the hospitalization and discharge letter including recognition and ability to use a computerized medical record.
8. Demonstrate prudent use of online information sources, be able to perform a critical assessment of the relevant literature and be able to make diagnostic and therapeutic decision and convey this information to the professionals, the patient and his family members.
9. Conduct effective communication while maintaining respect for colleagues, staff, patients and their families . Using codes for ethical and thoughtful ways for expression and formulation.
10. Behaving professionally - maintaining boundaries, ethical conduct, teamwork, maintaining medical confidentiality and concern for the patient's safety.
11. Adjust the therapeutic approach to the culture of the patient and his family while promoting the provision of accessible, equitable and fair medical care.
Attendance requirements(%):
Full attendance is required. An absence that exceeds 20% will require completion of the study period.
Teaching arrangement and method of instruction:
Bedside teaching, filling out a medication anamnesis form for the patients discussed during the visit, submitting case analysis individually, team discussion, brainstorming of a group of students presenting a complex case, preparing lectures, presentations, repeating guidelines for drug treatment relevant to the departments . Assimilation of knowledge and guidelines for providing recommendations based on the most up-to-date research. Attendance at all departmental activities including Journal Club and lectures.

Lectures: giving lectures required as part of the course (departmental journal club, morning question (a focused topic delivered in 10 minutes),
Participation: To present in an active way during the course, participation in group discussions and brainstorming
Demonstrating curiosity, asking questions and constructive criticism for the purpose of promoting and efficiency of the educational process.
Academic progress in reading and understanding cases, analysis and decoding, writing recommendations, expressing a desire for consultation, self-criticism, and reading targeted literature while striving for accuracy and depth.
Other: submission of drug anamnesis pages or consultation pages for drug treatment or writing treatment protocols.
The evaluation grade will be given according to the categories of fail/pass/very good/excellent. A passing grade in a class evaluation is 60 (threshold value of "passing"). Each category earns "bonus" points for the final grade in the internal, as an addition to the grade on the lecture. Good - 2 points, very good - 5 points, excellent - 10 points, provided that the grade in the final test is passing.
